Making your oc bio

First off, making a bio for your oc is not a necessity if you already posted it on the deviantart group and got accepted, but you can still post it here if you wish to make the ref here 
Here's the information that will be needed
Note that you can only make 1 dema!
Here's the information that will be needed- If you are making a Shiniko:

[b]Age of death :[/b]
[b]House :[/b]
[b]House ability :[/b]
[b]Abilities :[/b]
[b]Other forms :[/b]
[b]Personality :[/b]
[b]History :[/b] (It's more to know how your character died, you don't need to write up all the story since most shiniko's dont remember their pasts, only their dema's can see it, but just need to know how they died)
[b]Physic description :[/b] (You can send a drawing if you drew said oc or write it up if you wish)
[b]Other facts :[/b]
- If you are making a Phantom or Afflicted:

[b]Gender :[/b]
[b]Age of death :[/b]
[b]Former house :[/b] (If they had one)
[b]Abilities :[/b]

[b]Shiniko's :[/b] (If your dema has shiniko's, list them here and mention the space you got left, but if your dema doesn't have any just let in 'None - 20/20 space left, one of the mods will modify it whenever you have shinikos)
[b]Affinities :[/b] (Affinities are the types of powers they're best at, but demas are technically capable of learning most of what other teams can do, just not to the extent of one that is naturally inclined)

[b]Physic description :[/b] (You can send a drawing if you drew said oc or write it up if you wish. You gotta describe both his true form and human form)
